4 items, administered verbally by nurse or clinician to youth (preferably without parent present) in approximately 20 seconds. Developed by the National Institute for Mental Health to prevent suicides in youth, the free toolkit includes the ASQ Suicide Risk Screening Tool (NIMH) (PDF Document 208 KB) as well as follow-up resources for positive screens (defined as one or more positive responses). For use in medical settings (emergency department, inpatient medical/surgical units, outpatient clinics/primary care). Sensitivity 97%, specificity 88% in an emergency department validation study.
